MyHealth is an Internet service that allows Essentia Health patients to have secure Web-based access to portions of their Essentia Health electronic medical record and allows secure electronic messaging with Essentia Health. Use of MyHealth is completely voluntary, and you are not required to request access or to use it.
To obtain a MyHealth account, you must take the following steps: (1) Request access. Access can be requested in writing using the proper MyHealth authorization form or through completion of an on-line request. You must provide us with an email address as part of the request. (2) After Essentia Health approves your request, you will receive a temporary, one-time use activation code. From the date the activation code is issued, you will have ONLY 60 days to log-on and complete the activation process. After 60 days, if you have not activated your account, your temporary code will expire. Once the code expires, you will be asked to start the sign-up process a second time.
You must agree to the following terms and conditions before being given access. Please read this information carefully. By accessing or using MyHealth you agree to be bound by all of these terms and conditions. It is important to review the terms and conditions periodically as we may modify them at any time, and you agree that such modifications are effective and binding upon you immediately upon posting of the modified version.
Essentia Health offers MyHealth to its patients to help them keep track of certain information about their health and to communicate more easily with their health care providers at Essentia Health about certain aspects of their medical care. Essentia Health does not guarantee the accuracy of the information on the MyHealth site. It could contain typographical errors, inaccuracies, or other errors. If you see an error, please inform Essentia Health by contacting your physician's office so that it can be reviewed. Please be aware that information contained on the MyHealth site may be changed or updated without notice.
Please note that, to protect your privacy, only a limited amount of information regarding your health care and condition is available in MyHealth. Certain test results, diagnoses, conditions, and other information may not be included in your MyHealth record. Posting of information is at the discretion of your individual physician(s). MyHealth is not a substitute for your own personal consultation with your physician or other health care provider regarding your condition and how the content on this site may or may not apply to you. You may also request a more complete copy of your medical record through Essentia Health Release of Information offices. You should be aware that any messages you send or entries you make through MyHealth may become part of your permanent medical record.
Urgent Medical Matters
Essentia Health staff may not be immediately available to respond to your MyHealth messages and requests, therefore you should never use MyHealth electronic communications in an urgent situation or emergency. You should contact your physician’s office by phone, go to a hospital emergency room, or call 911.
If you have a MyHealth account you may choose to communicate with your primary care physician, and request appointments and pharmacy refills. You may also choose to be notified by e-mail when there is new information in your account and responses to the messages/requests you have sent. This means that anyone with access to your e-mail -- perhaps your employer, your spouse, or anyone else you allow to see your e-mail -- will be able to see this notification. Although the notification will not contain any private medical information, the fact that new medical information about you is available is something that you might not want others to know. You should always remember to update your e-mail address for MyHealth if it changes for any reason. Essentia Health cannot provide advice regarding another person’s healthcare, in response to questions asked through your MyHealth account.
Privacy and Security
Essentia Health is committed to protecting your personal medical information through the use of detailed policies, staff education and technical tools, such as secure internet connections and encryption. These measures conform to state and federal laws governing the confidentiality and security of medical records.
There are things that only you can do to protect the confidentiality of your information in MyHealth. You will be issued a temporary activation code and use a unique username and password to access your information in MyHealth. It is your responsibility to keep the activation code, username and password confidential. Anyone with access to your activation code, username and password may be able to access your confidential medical information. If at any time you feel that your password is no longer confidential, you can change your password and password hint online at any time. An unauthorized person may also see your confidential medical information if you fail to take certain other steps to protect it. For example, not logging out of an active MyHealth session, not protecting information you may print from MyHealth, or using personal computer settings or installed software products that may compromise information security, all may lead to unintended disclosure of your information.
You understand that requests will be routed to the appropriate Essentia Health department as necessary for handling, and therefore Essentia Health employees, other than your health care provider may be involved in addressing your request. Therefore, use of MyHealth may not be appropriate if there is sensitive information that you would only wish to share directly with your provider.
If you wish to access another person’s medical information through MyHealth, you may request proxy access by completing the proxy access form and submitting it to Essentia Health. You must have your own active MyHealth account for proxy access. In the event an individual is granted proxy access for an adult over the age of 18 or advanced access for minors between the ages of 12 and 18, the proxy access expires on an annual basis and can be renewed by completing a new proxy access form.
Parents and legal guardians of minors under the age of 18 may request proxy access to the child's MyHealth account. Only those with legal rights to access a child's medical information will be allowed proxy access. A proxy must notify Essentia Health immediately if his/her legal relationship with the child changes. Because of laws protecting the confidentiality of certain medical information of minors, full proxy access to MyHealth information will terminate on the child's 12th birthday, unless the child authorizes advanced proxy access. Partial proxy access to MyHealth information, including immunization records, allergy listings, appointment scheduling requests, and messages to your child's physician's office, will be provided for children aged 12 to 18 and will terminate on the child's 18th birthday.
Essentia Health may choose to terminate proxy access at any time for any reason.
Ownership and Restrictions on Use
All the content included in MyHealth, including, but not limited to: information, text, photographs, logos, icons and other marks in MyHealth are the property of Essentia Health and/or its subsidiaries or affiliates, or the third parties that own them, and are protected by copyright and trademark law. No user may reproduce or use such information, logos, icons or marks without the prior written consent of the owner. MyChart® is a registered trademark of Epic Systems Corporation.

MyHealth may provide links to other medical Web sites not managed by Essentia Health. The links are provided for general information and convenience only. Essentia Health does not endorse them, and has not verified the accuracy of the information on these sites, or whether any of that information applies to you and your medical condition. Use of others sites is strictly at your own risk including, but not limited to, any risks associated with destructive viruses.
Rights of Essentia Health
Essentia Health may terminate access to any MyHealth account at any time, for any reason. You will be notified if access has been terminated. MyHealth may not be available all the time, because of system failure, repair procedures, maintenance, or other reasons as determined by Essentia Health. MyHealth is provided on an "as-is" basis, and Essentia Health does not take any responsibility for, and will not be liable for, any inaccuracies or defects in MyHealth material or information or in software, communication devices, virtual private networks, the internet, any internet service provider, access system, computer hardware or software, or any other service or device, or a user's inability to access MyHealth for any reason.
By using MyHealth, you agree that the laws of Minnesota, without regard to principles of conflict of laws, will govern these Terms and Conditions and any dispute that might arise between you and Essentia Health, its doctors, nurses, staff or other affiliates. You expressly agree that exclusive jurisdiction for any dispute with Essentia Health, its affiliates, employees, subsidiaries, contractors, officers and directors, resides in the courts of St. Louis County, Minnesota and expressly consent to the exercise of personal jurisdiction in the in the courts of St. Louis County, Minnesota in connection with any claim involving Essentia Health, its affiliates, employees, subsidiaries, contractors, officers and MyChart.
