  Send a message to your doctor's office

MyHealth allows you to send a secure message about a non-urgent medical concern to your doctor's office. The message is received and responded to by your doctor's care team (nursing staff and advanced practitioner) and if needed, will be sent to your doctor for more information. You will get a response within two (2) business days.

E-mails are first received by your doctor's office staff, just like a phone call. Nurses or support staff will promptly answer your questions whenever possible. They will forward your questions to your doctor when needed. Any message sent to your doctor's office through MyHealth becomes a permanent part of your electronic medical record.



  Schedule an appointment


MyHealth allows you to view appointments available with your physician or advanced practitioner. This feature is available for primary care, family medicine, internal medicine and pediatrics. Once you find an appointment, you can schedule it directly online.

MyHealth also allows you to request an appointment with a specialty care physician or advanced practitioner that you have seen in the last three (3) years.

  Refill your Prescriptions


Refill your prescription for pick-up at any Essentia Health pharmacy. To refill your prescription, click on Request a refill next to the name of the medicine you wish to refill.



  View upcoming appointments


View details about any upcoming appointments including special instructions for the appointment and driving directions to the clinic.




  View your test results

Most lab results from clinic visits are placed in MyHealth early the next day. Lab results from hospital stays are placed in MyHealth when you leave the hospital.
Radiology and pathology results are placed in your MyHealth account four (4) business days after the results are final.

Please note: To protect your privacy, certain test results, diagnoses, conditions and other information may not be included in your MyHealth record. Your doctor(s) will decide what information should be posted in your MyHealth account.



  View your visit notes


Visit notes are written by your physician, advanced practitioner and/or nurses following your visit. These notes are a review of your visit and the care plan you and your healthcare team discussed.




  Access Preventive Care Information

Preventive medicine plays an important part in your health and overall well-being. View a list of procedures or immunizations that are recommended for people of your age, gender and medical history.

Click on: Medical Record tab; under My Record, select Preventative Care.

For any procedures for which you are overdue, you can select the checkbox and choose Request Appointment.

  View Immunizations


View a list of your immunizations. Also, you can view the immunizations recommended based on age, gender and medical history. A complete list of your immunization history can be printed.

Click on: Medical Record tab; under My Record select Immunizations.


  View your child's or another person's MyHealth account (proxy access)

You can access your children's or another adult's health records. This is called proxy access. Depending on the age of the person whose record you would like to view, the authorizations that are required are different. Proxy access allows you to send messages and schedule appointments for the person you care for.

Once you have been given proxy access, you will see that person's name on a tab on the right side of your MyHealth account. Click on their name to access their MyHealth account.

To request access to another person's account, clink on the home screen button or visit the proxy tab above.



  EyeCare Center


    Access a printable summary of your last contact or eyeglass prescription.

   Click on: Medical Record tab; under EyeCare Center select either Last Eyeglass Prescription or     Last Contacts Prescription.



  Cancel appointments


You have the ability to cancel upcoming appointments through MyHealth.

Click on: Appointments tab; under Future Appointments select Cancel Appt.



  Review & update your medications

A complete medication list, including prescriptions, supplements and over-the-counter medications, is available in MyHealth. If you see something is accurate, you can submit a change to this list through MyHealth.

Click on: Medical Record tab; under My Record select Medications.


  View your current health issues

Your health issues are a list of diseases or conditions you have discussed with various members of your health care team. This list allows you to link to trusted health information through Medline Plus to learn more about a specific condition.

Click on: Medical Record tab; under My Record select Current Health Issues.

  View your health summary

A health summary includes your current health issues, medications, allergies, immunizations and preventative care reminders all in one location.

Click on: Medical Record tab; under My Record select Health Summary.



  What is Essentia MyHealth?

MyHealth offers patients personalized and secure online access to portions of their medical records.

With MyHealth, you can:

How do I sign-up?

Patients have two ways to become a MyHealth user:

  1. Ask to sign up in your doctor's office. You will sign an authorization form and set up a user ID and password
  2. Go to EssentiaMyHealth.org and click "No Access Code? Request access now." You will complete an electronic authorization and you will receive an activation code and directions for getting started via US Mail. Use this code to sign up and create your own User ID and password.


Is my access code my user ID?

No. You will use your access code to log in to MyHealth for the first time. It expires after that one-time login or after 60 days if you do not use it. Once you log in successfully, you will create your User ID and password.

You will need to use your User ID and password each time you access MyHealth. Please keep your ID and password safe and private. Your MyHealth User ID and password will not expire as long as you remember them correctly. If you forget your User ID or password, please call the MyHealth Patient Support Line at 1.888.868.9292.

Privacy of Your Health Care Information

Your personal medical information in MyHealth is subject to all state and federal laws governing the confidentiality and security of medical records. Essentia Health is committed to protecting your information through means such as passwords, encryption, and audits. When you access your own information in MyHealth, it is through a secure connection.

MyHealth is owned and operated by Essentia Health and is fully compliant with federal and state laws pertaining to your privacy.

Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by Essentia Health.

Is MyHealth secure?

Privacy of your health information is our prime concern. Secure access codes, personal ID's and passwords (controlled by you) all help to keep your information safe. MyHealth uses the latest encryption technology to make sure that only you can access your account. MyHealth messaging is not like regular e-mail. Your MyHealth messages are sent while you are securely logged on to our website. This keeps your messages to your healthcare team safe.

Is my privacy protected?

Essentia MyHealth is a secure Internet site and your information is protected under the same privacy laws that guard your medical records. Only you, and those you have authorized, have access to your information. We use the latest safeguards including firewalls, passwords and encryption to protect your information.

There are things that only you can do to protect the confidentiality of your information in MyHealth, such as keeping your activation code, user name and password completely confidential and logging out of MyHealth before walking away from your computer.

Who do I contact if I have a question about information I see on MyHealth?

Essentia MyHealth pulls information directly from your electronic medical record. If you have a question about information in your MyHealth account, contact your doctor's office.

MyHealth messages are not for urgent health issues. If your concern is urgent, call your doctor's office,
go to a hospital emergency room or call 911.

  Proxy Access

Can I access my children's or another adult's health information in MyHealth?

Yes, if that person signs a proxy access authorization form (children under age 12 do not need to sign a form). This document makes you their proxy and gives you access to their account. You can complete a proxy authorization form at any Essentia Health clinic or hospital, or you can log into your MyHealth account, go to the "Preferences" tab on the MyHealth home page and select ''Personalize and Proxy Access''.

A proxy access authorization form must be submitted and approved before access will be granted. Click the ''Proxy/Caregiver Access'' tab above for more information.

Why can't I see all of my teenager's MyHealth account information?

Some of your teenager's health information is protected by state law. You will be able to view immunization and allergy records, request appointments and send e-mail messages to your child's doctor's office. Full access to their account will be granted if the teen signs the proxy authorization form.

Can I ask questions about a family member from my own MyHealth account?

No. You should never use your MyHealth account to ask advice about another person. The information you give will become part of your own permanent medical record. This could be very harmful both to the other person and to your health care.

  Technical Support

Where can I update my personal information (such as change my password or update my e-mail address)?

Once you have logged into your MyHealth account, you can update your email address or change your password using the "My Preferences" tab on the MyHealth Home page.

Can I use any computer to access MyHealth?

After you receive an access code or a User ID and password, you can use any computer or smart device that has Internet access to view your MyHealth account.

What do I need to use MyHealth?

You will need your User ID, password and access to a browser (such as Safari, Internet Explorer, Firefox or Chrome). You may use either a computer or a smart device to access MyHealth.

What if the MyHealth site doesn't look or work right?

If boxes, tabs or text don't line up correctly or slide off the page, you may need to update your browser. MyHealth is best run on Internet Explorer 8 or above and the most recent versions of Safari, Firefox and Chrome. To update Internet Explorer, you can get it here

I am not receiving e-mails from MyHealth, what should I do?

If emails from the MyHealth website do not reach your Inbox, check your junk or spam mail folder. Sometimes e-mail filters will send messages from MyHealth to your junk mail folder. In order to keep this from happening, be sure to add MyHealth to your list of allowed senders.

  Browser Support

I was logged out of MyHealth, what happened?

To protect your privacy and the security of your information, your MyHealth session ends if your keyboard remains idle for 20 minutes. It is recommended that you log out of your MyHealth session if you need to leave your computer for even a short period of time.

I am experiencing technical problems, what can I do?

If you are experiencing problems such as being logged out immediately after logging in, an extension or add-on in your web browser may be causing problems with MyHealth. Use the links below to learn how to disable extensions and add-ons in some of the most popular web browsers.

If disabling extensions and add-ons does not resolve your problem, try clearing "Cookies" in your web browser using the instructions below.

Finally, if you are still having problems, try installing a different web browser. There are links below to download the latest version of each web browser.

MyHealth® Terms and Conditions

MyHealth is an Internet service that allows Essentia Health patients to have secure Web-based access to portions of their Essentia Health electronic medical record and allows secure electronic messaging with Essentia Health. Use of MyHealth is completely voluntary, and you are not required to request access or to use it.

To obtain a MyHealth account, you must take the following steps: (1) Request access. Access can be requested in writing using the proper MyHealth authorization form or through completion of an on-line request. You must provide us with an email address as part of the request. (2) After Essentia Health approves your request, you will receive a temporary, one-time use activation code. From the date the activation code is issued, you will have ONLY 60 days to log-on and complete the activation process. After 60 days, if you have not activated your account, your temporary code will expire. Once the code expires, you will be asked to start the sign-up process a second time.

You must agree to the following terms and conditions before being given access. Please read this information carefully. By accessing or using MyHealth you agree to be bound by all of these terms and conditions. It is important to review the terms and conditions periodically as we may modify them at any time, and you agree that such modifications are effective and binding upon you immediately upon posting of the modified version.


Medical Information

Essentia Health offers MyHealth to its patients to help them keep track of certain information about their health and to communicate more easily with their health care providers at Essentia Health about certain aspects of their medical care. Essentia Health does not guarantee the accuracy of the information on the MyHealth site. It could contain typographical errors, inaccuracies, or other errors. If you see an error, please inform Essentia Health by contacting your physician's office so that it can be reviewed. Please be aware that information contained on the MyHealth site may be changed or updated without notice.

Please note that, to protect your privacy, only a limited amount of information regarding your health care and condition is available in MyHealth. Certain test results, diagnoses, conditions, and other information may not be included in your MyHealth record. Posting of information is at the discretion of your individual physician(s). MyHealth is not a substitute for your own personal consultation with your physician or other health care provider regarding your condition and how the content on this site may or may not apply to you. You may also request a more complete copy of your medical record through Essentia Health Release of Information offices. You should be aware that any messages you send or entries you make through MyHealth may become part of your permanent medical record.

Urgent Medical Matters

Essentia Health staff may not be immediately available to respond to your MyHealth messages and requests, therefore you should never use MyHealth electronic communications in an urgent situation or emergency. You should contact your physician's office by phone, go to a hospital emergency room, or call 911.

Electronic Communication

If you have a MyHealth account you may choose to communicate with your primary care physician, and request appointments and pharmacy refills. You may also choose to be notified by e-mail when there is new information in your account and responses to the messages/requests you have sent. This means that anyone with access to your e-mail -- perhaps your employer, your spouse, or anyone else you allow to see your e-mail -- will be able to see this notification. Although the notification will not contain any private medical information, the fact that new medical information about you is available is something that you might not want others to know. You should always remember to update your e-mail address for MyHealth if it changes for any reason. Essentia Health cannot provide advice regarding another person's healthcare, in response to questions asked through your MyHealth account.

Privacy and Security

Essentia Health is committed to protecting your personal medical information through the use of detailed policies, staff education and technical tools, such as secure internet connections and encryption. These measures conform to state and federal laws governing the confidentiality and security of medical records.

There are things that only you can do to protect the confidentiality of your information in MyHealth. You will be issued a temporary activation code and use a unique username and password to access your information in MyHealth. It is your responsibility to keep the activation code, username and password confidential. Anyone with access to your activation code, username and password may be able to access your confidential medical information. If at any time you feel that your password is no longer confidential, you can change your password and password hint online at any time. An unauthorized person may also see your confidential medical information if you fail to take certain other steps to protect it. For example, not logging out of an active MyHealth session, not protecting information you may print from MyHealth, or using personal computer settings or installed software products that may compromise information security, all may lead to unintended disclosure of your information.

You understand that requests will be routed to the appropriate Essentia Health department as necessary for handling, and therefore Essentia Health employees, other than your health care provider may be involved in addressing your request. Therefore, use of MyHealth may not be appropriate if there is sensitive information that you would only wish to share directly with your provider.

Proxy Access

If you wish to access another person's medical information through MyHealth, you may request proxy access by completing the proxy access form and submitting it to Essentia Health. You must have your own active MyHealth account for proxy access. In the event an individual is granted proxy access for an adult over the age of 18 or advanced access for minors between the ages of 12 and 18, the proxy access expires on an annual basis and can be renewed by completing a new proxy access form.

Parents and legal guardians of minors under the age of 18 may request proxy access to the child's MyHealth account. Only those with legal rights to access a child's medical information will be allowed proxy access. A proxy must notify Essentia Health immediately if his/her legal relationship with the child changes. Because of laws protecting the confidentiality of certain medical information of minors, full proxy access to MyHealth information will terminate on the child's 12th birthday, unless the child authorizes advanced proxy access. Partial proxy access to MyHealth information, including immunization records, allergy listings, appointment scheduling requests, and messages to your child's physician's office, will be provided for children aged 12 to 18 and will terminate on the child's 18th birthday.

Essentia Health may choose to terminate proxy access at any time for any reason.

Ownership and Restrictions on Use

All the content included in MyHealth, including, but not limited to: information, text, photographs, logos, icons and other marks in MyHealth are the property of Essentia Health and/or its subsidiaries or affiliates, or the third parties that own them, and are protected by copyright and trademark law. No user may reproduce or use such information, logos, icons or marks without the prior written consent of the owner. MyChart® is a registered trademark of Epic Systems Corporation.

MyHealth may provide links to other medical Web sites not managed by Essentia Health. The links are provided for general information and convenience only. Rights of Essentia Health

Essentia Health may terminate access to any MyHealth account at any time, for any reason. You will be notified if access has been terminated. MyHealth may not be available all the time, because of system failure, repair procedures, maintenance, or other reasons as determined by Essentia Health. MyHealth is provided on an "as-is" basis, and Essentia Health does not take any responsibility for, and will not be liable for, any inaccuracies or defects in MyHealth material or information or in software, communication devices, virtual private networks, the internet, any internet service provider, access system, computer hardware or software, or any other service or device, or a user's inability to access MyHealth for any reason.

Other

By using MyHealth, you agree that the laws of Minnesota, without regard to principles of conflict of laws, will govern these Terms and Conditions and any dispute that might arise between you and Essentia Health, its doctors, nurses, staff or other affiliates.
